POLICY – SJCPMM-DM-001: SERVICE REQUEST

All request for services through the Saint James Documentation Ministry must be submitted in advance through the signed and dated completion of form SJCPMM-DM-FORM-001.1.  Blank copies of this form are now located on the Saint James Parish website.  Please open, print, fill out and return to the Parish Offices, attention Documentation Ministry.  You can also scan in a completed form and submit via email to itsupport@stjamescatholicparish.org.  Please note that if any minors 16 years of age or younger are involved, a Photo/Video Waiver Release form must also be completed, signed and returned with this request.

The planned photoing or videoing of minors 16 years of age and/or younger shall require the completion, advanced submittal and Ministry approval of attached form SJCPMM-DM-Form-002.1.  Please open, print, fill out and return to the Parish Offices, attention Documentation Ministry.  You can also scan in a completed form and submit via email to itsupport@stjamescatholicparish.org.  These completed forms must be submitted in advance with any “Service Request”.
